I choose to take part in this bead-making activity. I will follow the instructor’s directions and ask for help when needed. I may decline a step or stop participating at any time.

Reading glasses

Please bring your reading glasses if you need them for close-up work.

Small parts and material sensitivities

I understand that beads and charms are small parts and should be kept away from mouths, young children, and pets. Jewelry materials may cause irritation or allergic reactions in sensitive individuals. I will tell the instructor about relevant sensitivities or support needs before starting. Materials are not guaranteed allergen-free.

Signing this form does not give permission to use my photo or video for promotion. Photo permission is requested separately and is optional.
